To become a participating provider in the Prime Healthcare Network
- Requests to join the Prime Healthcare network can be done online by clicking here. Once you’re in the Provider Updates page, click on “Request a New Contract” and fill out the necessary information.

NOTE: If you are an existing network provider or group requesting updates or changes, the Provider Updates page can also be used to make changes to demographics, adding or removing providers to your group, making changes to member assignments (such as age restrictions, seeing new members), and updating NPI and tax information.
